As there are often multiple plans within insurance companies, it is the patient's responsibility to ensure that our doctors are participating within a specific plan before making an appointment.

As we make every effort to accommodate patients, 
please be aware of the following practice policies: 


APPOINTMENT CANCELLATION :

We have a new Google appointment confirmation line -

ALL appointments MUST be confirmed 1-3 days prior to your appointment day. We will attempt to reach you, but if your appointment is not confirmed, it may be forfeited & given to another patient on our waiting list.

It is your responsibility to call our office , a minimum of 24 hours prior to your appointment time in order to cancel.

Patients who confirm their appointments but do not show, will be billed $50.00. We reserve the right to discontinue patients who do not follow this policy.

Bring to your appointment:
  1. insurance card
  2. picture ID
  3. copay (cash or check only)
  4. all medications
  5. any past medical records or lab reports you have

Co-payments must be made by check or cash ONLY prior to your appointment.

We CANNOT accept credit or debit cards.
