As there are often multiple plans within insurance companies, it is the patient's responsibility to ensure that our doctors are participating within a specific plan before making an appointment.

APPOINTMENT CANCELLATION :

We have a new appointment confirmation line 

ALL appointments MUST be confirmed 1-3 days prior to your appointment day. 

TEXT or leave a Voice Mail Confirming or Cancelling your Appointment:  720-446-9742. 


We will attempt to reach you to confirm your appointments, but if your appointment is not confirmed, it may be forfeited & given to another patient on our waiting list.

If you fail to show up for your confirmed appointment 3 times, we reserve the right to terminate our professional relationship as your primary care provider.

You are responsible for providing our office with a reliable phone number and email address in which you can be reached.

IF YOU ARE A NEW PATIENT:

Your completed new patient forms imust be received 48 hours in advance of your appointment in order for us to build your medical record and prepare for your appointment.

PLEASE NOTIFY US IMMEDIATELY IF YOUR INSURANCE CHANGES!
Co-payments must be made by check or cash ONLY prior to your appointment.

We CANNOT accept credit or debit cards.

Cash prices for patients without health insurance are posted in our office.
